What does breast implant malposition mean?
Breast implant malposition means that an implant is not sitting in the intended relationship to the breast fold, chest wall or surrounding tissues. It may move downward, outward, inward or in another direction. The result can be a change in breast shape, spacing, lower-pole length or the position of the nipple relative to the implant.
If you are researching breast implant malposition, do not diagnose it from a single photograph. Early swelling and settling can change the appearance, while a persistent contour may have more than one cause. A qualified plastic surgeon must examine the breast, compare both sides and decide whether imaging or a revision discussion is appropriate.
What is bottoming out?
Bottoming out generally describes downward displacement of the implant and lengthening of the lower pole. The breast fold may sit lower, the implant may appear too low and the nipple can look relatively high on the breast. A weak or stretched lower soft-tissue boundary, an implant that is too large for the tissue, or changes to the fold can contribute, but the cause is individual.
Not every implant that looks high or low soon after surgery is malpositioned. Implants can appear high while swelling settles, and the two sides may settle at different speeds. The concern is a persistent or worsening relationship between the implant, fold and nipple, especially when there is discomfort, asymmetry or a visible contour change.
What is lateral displacement?
Lateral displacement means that the implant sits too far toward the outside of the chest. A patient may notice a wider gap between the breasts, fullness under the arm or an implant that moves outward when lying down. Natural chest width, pocket boundaries, muscle movement, tissue support and implant dimensions can all influence the appearance.
Inward displacement is another possibility. Excessive movement toward the middle can reduce the natural space between the breasts and, in severe cases, create symmastia, sometimes called “uniboob”. A change in the visible border does not always mean the implant has travelled as a single solid object. The breast tissue can stretch, the fold can descend, or the implant can settle within a pocket that has changed. This distinction matters because revision should address the structure that is actually causing the contour, not simply replace a device without examining the pocket and soft tissues.
What is a double bubble?
A double bubble is a step or two-contour appearance where the implant’s lower contour and the natural breast fold do not align. It can be seen when the original breast has a constricted or low fold, when the implant settles below the fold, or when a prior breast shape and the implant create separate curves. A similar appearance can also come from natural anatomy rather than a single technical error.
Double bubble is not diagnosed by a keyword or photograph. The surgeon considers the position of the implant, the old and current fold, the lower-pole tissue, nipple level, breast shape and any previous surgery. A breast lift, implant change, pocket repair or another procedure may be discussed in selected cases, but no single correction fits every patient.
Why can malposition develop?
Malposition can relate to the original anatomy, implant dimensions, the pocket, tissue elasticity, healing, trauma, weight change or a later change in the breast. A capsule may tighten or the surrounding tissue may stretch. A large or heavy implant can place different demands on the soft-tissue envelope from a smaller device, but size alone does not explain every case.
Timing matters. A change immediately after surgery has a different differential from a change that appears years later. Pain, redness, swelling, fever, a mass or sudden change requires medical attention because infection, fluid, rupture or another complication may be present. Do not assume that malposition is the only explanation.
What questions should you ask about assessment and revision?
- Is the implant truly outside the intended position, or could the appearance be early settling or a change in the natural breast?
- Which boundary has changed: the fold, outer breast, inner breast or upper contour?
- Could implant size, width, projection, capsule, tissue quality or previous surgery be contributing?
- Would ultrasound, MRI or another test help rule out rupture, fluid or a different problem?
- Would revision involve pocket repair, implant exchange, support of the fold, a lift or another procedure, and what recurrence risk remains?
Revision for malposition is a structural decision, not a promise that one photograph can be recreated. The aim is to improve the relationship between the implant and your tissues while respecting the limits of the chest.
