Smooth vs textured breast implants refers to the surface of the outer shell. A smooth implant has a less textured shell that can move within the breast pocket. It is not breast cancer. The FDA states that the risk is higher for textured surface implants than for smooth surface implants, although the level of risk is not the same for every textured product and the overall condition is uncommon.

This is important information, but it should be communicated accurately. A higher association with textured surfaces does not mean that every patient with a textured implant will develop BIA-ALCL, and a smooth implant is not a promise that no implant-related problem can occur. The FDA continues to distinguish between the risk associated with surface texture and other implant risks.

In its current patient communication, the FDA does not recommend routine removal of breast implants in patients who have no symptoms solely because of concern about BIA-ALCL. That does not remove the need for informed consent or clinical follow-up, and it does not mean that a personal decision about removal is never appropriate.

If you already have an implant, record its manufacturer, model, surface and date of placement if those details are available. Discuss any anxiety, device alert, new symptom or planned revision with your surgeon. Removal or replacement is itself a surgical decision with its own possible risks, so it should be based on your device, examination, symptoms and preferences.
